How Our Care Plans Are Developed

 

All of our nursing or therapeutic service programs begin with a doctor’s prescription. Initial Home Care plans are developed in-house. We then perform a full in-home assessment followed by a written plan reviewed by the patient and/or family, Director of Nursing, and Homewood Manager. All costs are determined and assistance is provided with eligibility requirements and funding approvals from third-party payors. Only upon final family approval are services implemented and written records of each visit are made and reviewed no less than weekly by management staff. Follow-up with families occurs twice a month or more often if necessary. Family requests for plan review are conducted immediately.
